Location: Coalinga, CAJob SummaryThe Director of Medical Records plans, organizes, and directs Medical Records Services. The Director of Medical Records ensures that coders are provided medical record and procedure log information so that timely, accurate concurrent coding can be accomplished. Responsible for timely record completion, placing dictated documentation in the correct record, ensuring that medical records are maintained in order including thinned records, and that files records are filed and stored according to policy and procedure. The Director of Medical Records complies with all regulatory and HIPAA policies.QualificationsThe position requires a Bachelor degree or credentialing as a Registered Health Information Technician (RHIT). One to three (1-3) years experience as a Manager or Assistant Manager of a Medical Records Department preferred. A minimum of 20 CEU's must be completed every 2 years. Good interpersonal skills are important.P. The duties and responsibilities are listed below. It is the expectation of the institution that the employee be able to demonstrate competency in each area. It is the duty of the institution to provide orientation and performance evaluation in each area stated.Duties and ResponsibilitiesASSEMBLES CHARTS IN PROPER ORDER, AND IN A TIMELY MANNERRecords are coded within three (3) days of discharge and physician queries are completed timely. Impact: PPS and financial management.ANALYZES DISCHARGED CHARTS FOR COMPLETENESS IN A TIMELY MANNERExplains important elements of an incomplete chart.Describes process of how to use the chart completion log.Files all loose reports weekly.MONITORING OF COMPLIANCE AND DOCUMENTED FOLLOW-UP WITH PHYSICIANS FOR THE PRESENCE OF A HP ON EVERY ACUTE PATIENT CHART WITHIN 24 HOURS OF ADMISSION. IMPACT: PATIENT CARE, ACCREDITATION, AND CMSASSISTS DEPARTMENT VISITORS IN OBTAINING CHARTS/INFORMATION IN A COURTEOUS MANNER.HANDLES/DIRECTS TELEPHONE COMMUNICATION WITH REGARD TO ALL ASPECTS OF THE MEDICAL RECORDS DEPARTMENT.Follows through with phone messages.MONITORS AND MAINTAINS CHART COMPLETION BY CLINICIANS AND PHYSICIANSSends out weekly notices to cliniciansMonitors/calls and/or sends letters to physicians bi-weeklyEvidence of appropriate measure to avoid total medical record delinquency that equals or exceeds twice the average monthly discharge rate. Appropriate measures include tracking all post-discharge physician deficiencies in CPSI and documented bi-monthly physician notification of incomplete records with CEO and Medical Director involvement. Impact: Accreditation requirement implying conditional accreditation, CMS, patient care.PERFORMS RELEASE OF INFORMATION DUTIES IN AN ACCURATE AND TIMELY MANNER.Demonstrates correct use of the release of information log.Compliance with HIPAA policies and procedures including use and disclosure of protected health information. Evidence of compliance includes release of medical records with proper authorization when needed and physical security of medical records. Impact: Federal law, patient rightsMAINTAINS/PREPARES AND SUBMITS ALL ASSIGNED REPORTS IN AN ACCURATE AND COMPLETE MANNER MEETING ALL DEADLINESCREDENTIALINGThere is one acknowledgement statement located in each credentialing file that is signed by the physician prior to seeing their first Central Valley Specialty Hospital patient.Physicians and allied health are appointed and reappointed in accordance with hospital bylaws and credentialing policies. Impact; Accreditation, CMSA physician roster and binder with current delineation of privileges is maintained and accessible to clinical staff. ** Impact: Accreditation, CMSExpirables such as license, malpractice insurance and DEA are monitored on a log and can be verified with evidence in the credentialing file.
