This specialized role offers the flexibility of part-time travel assignments, allowing you to bring expert care to patients in their homes and care facilities while enjoying a supportive work environment and competitive compensation. In this pivotal role, you will provide pediatric hospice home visits (4–5 visits per day), conduct assessments in skilled nursing and care facilities, and perform face-to-face evaluations for Medicare recertifications in collaboration with the Medical Director. Your expertise will also support education for clinical team members, contribute to patient follow-up, and participate in inpatient rounds when necessary. Flexibility is key—this position entails 1–2 days per week, with full or half-day scheduling options, Monday through Friday, during the day shift only. No evenings or weekends are required. Required Skills: Valid Nurse Practitioner license with pediatric hospice experience Proven experience conducting home visits and assessments in hospice settings Excellent communication and interpersonal skills Ability to collaborate effectively with multidisciplinary teams Strong organizational skills and attention to detail Knowledge of Medicare recertification processes Nice to Have Skills: Experience working with skilled nursing facilities and care homes Certification in hospice and palliative care Bilingual abilities to support diverse patient populations Experience with electronic medical records systems Preferred Education and Experience: Master’s Degree in Nursing (MSN or higher) Minimum of 2 years of hospice or palliative care experience, preferably in pediatric settings Prior travel or flexible scheduling experience is advantageous Other Requirements: Current state licensure and certifications as required Ability to travel to patient locations, including home, SNFs, and care facilities Valid driver’s license and reliable transportation Willingness to work full or half-day schedules as needed Compensation for this role is $81.74 per hour, with weekly pay via direct deposit.
