Outreach Worker*, you will make an *impact daily* on peoples lives
through a social connection that is not happening in the current
healthcare system.This is a *hands-on role*, which involves
*advocating* for patients, *facilitating access* to healthcare,
housing, and social services, and employing telehealth for expanded
reach. Your focus on patient engagement and education, along with
your deep cultural competence, ensures care is delivered
respectfully and effectively. The ideal candidate would be

you'll help improve health outcomes for all patients. This

well-being.*Responsibilities:** Showcase exemplary *communication*,
and organizational prowess to cultivate a vibrant and positive work
atmosphere.* Ability to *foster relationships*, prioritizing
patient-centered care, education, and organizational engagement.*
High levels of *empathy* to be able to engage with people with
various backgrounds.* Foster strong relationships with the *at-risk
population* in their environment.* Assists the Lead Care Manager
(LCM) and clinical team in *supporting patients* to comprehend care
plans and instructions, motivating them to *actively engage* in
their health journey. Diligently monitors services to ensure
*adherence to care plan goals*.* Continuously assesses the needs of
patients, identifying social determinants of health and
recommending appropriate follow-up and community connections.*
Coordinates patient care, *conducting home visits* and offering
culturally sensitive support for effective medical care and
behavioral change within the team.* Assists patients in accessing
resources, including appointment scheduling and navigating program
applications.* *Fosters positive relationships* with team members,
patients, providers, and community representatives to enhance
teamwork and service excellence.* Provides exceptional service to
all stakeholders, ensuring culturally and appropriate care,
attending meetings as necessary, and upholding established policies
and procedures while also undertaking additional duties and
projects as assigned.*Requirements:** The ability to *work
independently* with minimal supervision, coupled with *excellent
verbal and written communication* (in English), negotiation, and
relationship-building skills, is essential.* The role entails
*independent travel* for home visits; candidates must possess their
transportation, hold a valid California drivers license, maintain a
good driving record, and have vehicle insurance.* Proficiency in
utilizing computer and software, along with Microsoft Office
proficiency, is required for administrative tasks, research, and
professional networking.* A technical aptitude with strong
analytical, critical thinking, and reporting abilities is
necessary.* Effective interaction with community agencies,
patients, and clinical personnel is imperative.*Working
Conditions/Physical Requirements:** Requires high manual dexterity
and prolonged, extensive standing, sitting, walking, and lifting.*
Adequate hearing and clear speech for in-person or telephone
communication. Vision suitable for reading various documents,
including memos, screens, and forms.* Able to reach above the
shoulder level to work, must be able to bend, squat and sit, stand,
stoop, crouch, reach, kneel, twist/turn, etc.*Nice to haves:**
Experience working in a healthcare setting is preferred, but *not
required* as training will be provided - it is all about empathy
here.* *Familiarity *with the local community with lived
experiences where services are rendered is preferred.* Bilingual in
Spanish, Hmong or Punjabi is a strong plus.* CPR certification is
preferred.*Compensation:** This role offers a salary of 50-60k*Does
